29 CFR 1926.102(a)(2): Eye and face protection equipment required by this Part shall meet the requirements specified in American National Standards Institute, Z87.1-1968, Practice for Occupational and Educational Eye and Face Protection.  a. The employee was operating an electric jack hammer with glasses that did not meet the requirement of ANSI Z87.1, thereby exposing employees to a flying hazard.

Suitability of equipment for an identified purpose may be evidenced by listing, labeling, or certification for that identified purpose.  a. There was a series two 50 foot extension cords plugged into each other (daisy chained) to power the electric jack hammer, thereby exposing the employees to an electrical hazard.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage.  a. The flexible cord was run through the doorway where it was pinched by a metal frame door, thereby exposing employees to an electrical hazard.  b. The extension cord was damaged approximately ½ inch from the strain relief, thereby exposing employees to an electrical hazard.

29 CFR 1926.404(b)(1)(i): General. The employer shall use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors.  a. The extension cord plugged into the receptacle in building 111, was not plugged into a GFCI, thereby exposing employees to an electrical hazard.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a. An employee was observed working on the balcony of a second story apartment without guard rails or other forms of fall protection, thereby exposing the employee to a fall hazard of 15 feet.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  a. The employee was accessing and egressing a balcony from a ladder that was 2 feet below the landing, the employee was exposed to a fall hazard of 15 feet.
