29 CFR 1926.651(f): A warning system such as barricades, hand or mechanical signals, or stop logs was not utilized when mobile equipment was operated adjacent to an excavation, when such equipment was required to approach the edge of an excavation: Construction site: On or about November 10, 2015 and at times prior thereto, a bob-cat was being operated adjacent to the excavation without a warning system in place. This condition exposed the employee to a fall of 14-feet to the ground below.
