29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as follows:    (a)  G & L Russo Masonry, at 825 Gilpin Street, Denver, CO 80203: On and before November 13, 2015, the exposing employer failed to ensure that all working-level platforms of the scaffold were fully planked.  Specifically, employees were observed working on a scaffold removing brick from a chimney with only two 2" x 12" wooden planks as a working surface.  Condition exposed employees to fall hazards of approximately 25 feet from the scaffolds working surface.

29 CFR 1926.451(b)(3): The front edge of all platforms was not 14 inches or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling:  (a)  G & L Russo Masonry, at 825 Gilpin Street, Denver, CO 80203; On and before November 13, 2015, the exposing employer failed to ensure that the maximum distance from the face of the work being performed to scaffolds working surface was a maximum of 14 inches.    Condition exposed employees performing work off the scaffold to fall hazards of approximately 25 feet.

(a)  G & L Russo Masonry, at 825 Gilpin Street, Denver, CO 80203: On and before November 13, 2015, the exposing employer did not to ensure that direct access to or from the scaffold was no more than 14 inches horizontally and no more than 24 inches vertically from the roof of a two-story single family residence, where the employees were removing and replacing brick from a chimney.   Condition exposed the employees to fall heights of approximately 25 feet.
