29 CFR 1926.451(a)(1): Each scaffold and scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it. a) Jobsite: Employees conducting brick washing activities from the outrigger of a supported tubular welded frame scaffold, approximately 20 feet above ground level, were exposed to fall hazards in that the side brackets supporting the outrigger platform were not capable of supporting at least 4 times the maximum intended load applied to them. An employee working from the outrigger of the third working platform fell after the side bracket supporting the platform snapped at the ends connected to the scaffold uprights. Noted on or about 11/9/15.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: a) Jobsite: The employer did not have the supported tubular welded frame scaffolds and their components inspected for visible defects by a competent person, knowledgeable in part of the structural integrity of scaffolds as well as the degree of maintenance needed to maintain them, prior to each shift. A scaffold side bracket supporting a working platform failed causing an employee to fall approximately 20 feet. Noted on or about 11/9/15.
