29 CFR 1910.254(d)(9)(i): The operator should report any equipment defect or safety hazard to his supervisor and the use of the equipment shall be discontinued until its safety has been assured. Repairs shall be made only by qualified personnel. (a) Ford F250 - On or about November 12, 2015 employees were exposed to an electrical shock hazard due to the leads being damaged and having insulation exposed on a welding machine. Miller Bobcat 250 Welder/Generator had visible damaged wires on both the grounding (work) cable and the electrode cable yet it was taken out of service.

29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors shall be replaced. Joining lengths of work and electrode cables shall be done by the use of connecting means specifically intended for the purpose. The connecting means shall have insulation adequate for the service conditions. [55 FR 13709, Apr. 11, 1990; 61 FR 9227, March 7, 1996; 70 FR 53929, Sept. 13, 2005] (a) Ford F250 - On or about November 12, 2015 employees were exposed to an electrical shock hazard due to the leads being damaged and having insulation exposed on a Miller Bobcat 250 Welder/Generator. Both the grounding (work) cable and the electrode cable had wires exposed.
