29 CFR 1910.37(b)(1) Each exit route was not adequately lighted so that an employee with normal vision can see along the exit route:  (a) North Exit Door:  Employees were exposed to fire hazards as the flood lights that were attached to the Exit sign were not working.  This violation was abated during inspection. Abatement certification for this violation is not required.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a)  Employees were exposed to electrical and amputation hazards as there was no written lockout tagout program when working on the balers, conveyors and other pieces of equipment.  The lockout tagout program requirements include employee training, periodic inspections, and machine specific lockout tagout procedures.  Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:  (a) Tool Room:  Employees were exposed to electrical hazards as the area in front of the main breaker panel and subpanel had a box of semi parts and other items stored in front of them.   This violation was abated during inspection. Abatement certification for this violation is not required.
