29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one-fourth inch.  Employees using the grinding machine in steel room were exposed to struck-by hazards in that the distance between adjustable tongue guard and peripheral member at the top exceeded one-fourth inch.

29 CFR 1910.215(a)(4): Work rests on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  Employees engaged in production activities using the grinder in the steel room were exposed to struck by hazards in that the distance between abrasive wheel and work rest was approximately three inches.  The Reliable Concrete Products and Ornamental Iron, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.215(a)(4) which was contained in OSHA inspection number 31603579, citation number 01, item number 003 and was affirmed as a final order on February 27th, 2012 with respect to a workplace located at 615 Scott Avenue, Kansas City, KS 66119.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards or specific chemicals:  Employees engaged in mixing, pouring and cleaning concrete products were exposed to chemical hazards in that training to recognize and to minimize the hazards associated with chemicals used was not provided.  The Reliable Concrete Products and Ornamental Iron, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(h)(1) which was contained in OSHA inspection number 316035179, citation number 01, item number 008(d) and was affirmed as a final order on February 27, 2012 with respect to a workplace located at 615 Scott Avenue, Kansas City, KS 66105.
