29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) Jobsite - Employees engaged in framing operations were exposed to a fall hazard greater than (10) feet above the grade without fall protection. The employer did not utilize a personal fall protection system, guard rails or a safety net system to protect employees from falling. Observed on or about 29 October 2015.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers:  a) Jobsite - Employees engaged in framing operations were exposed to a fall hazards through unprotected floor holes in the walking/working surface at heights greater than six (6) feet above the grade without fall protection. Observed on or about 29 October 2015.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Jobsite -  Ladders used to gain access to upper working levels did not extent at least 3 feet thus exposing employees to fall hazards. Observed on or about 29 October 2015.
