29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded: (a) On or about 11/04/2015 - Building 2, Cincinnati Press Brake # 9, the machine was operated without the point of operation guarded exposing employees to amputation hazards when the press brake was cycled. Elixir Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 314278110, citation number 1, item number 1 and was affirmed as a final order on September 6, 2011, with respect to a workplace located at 5500 Industrial Drive, Waco, Texas 76710.
