29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: Rear of Building 64: Employees were walking on the ground performing debris clean-up operations while employees on the roof were dropping roof debris from approximately thirty (30) feet above them. The employees were not wearing protective helmets exposing them to struck-by hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed Rear of Building 64: Employees were using an aluminum extension ladder to access the roof where the ladder did not extend three (3) feet above the landing exposing employees to a fall hazard of approximately thirty (30) feet.
