29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.    The employer's lockout/tagout program did not include documented machine-specific procedures for servicing and maintaining production machines and equipment including the Accurshears and Chicago Press located in the establishment. This exposed employees to caught-in hazards.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.    The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed. 29 CFR 1910.217(c)(1)(i): It shall be the responsibility of the employer to provide and insure the usage of "point of operation guards" or properly applied and adjusted point of operation devices on every operation performed on a mechanical power press. See Table O-10.    On or about 19 November 2015, the employer failed to ensure that point of operation guards and point of operation devices were applied for every operation performed on mechanical power presses including:    a. Minster #68. Front, rear, and side guarding on the press allowed employees to gain access to the point of operation. This exposed employees to a caught in and crushing hazard.    b. U.S. Baird #70. Machine guarding on the press allowed employees to gain access. The guarding on the front of the machine is left open to allow parts to fall out into buckets. Safety interlocks on the safety doors were bypassed or inoperable. This exposed employees to a caught in and crushing hazard.    c. Platarg #56. Interlock door was bypassed, which allowed the machine to operate when the safety interlocked door was open. This exposed employees to a caught in and crushing hazard.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d)(2): Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage.    During die changes and adjustments the employer did not ensure all energy sources were isolated or rendered inoperative. This exposed employees to a crushing hazard.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years.  The employer failed to evaluate each powered industrial truck operator's performance. Evaluations  shall be conducted at least once every three years. An evaluation was not conducted as required.  Abatement documentation is not required for this item.
