29 CFR 1926.1051(a): A stairway or ladder was not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided. a. On November 12, 2015, on the mid level deck (bridge area), an ironworker climbed the steel beam about 6 feet up to the next level beam without a ladder, exposing himself to a 26 feet fall hazard.

29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems: a. On November 12, 2015, on the mid level steel decking (bridge deck), employees involved in the steel erection process were not protected from a 20 feet fall hazard by the use of a fall protection system. b. On November 16, 2015, on the mid level steel decking (east deck), employees involved in the steel erection process were not protected from a 20 feet fall hazard by the use of fall protection system. Cutting Edge Welding and Fabrication was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.760(a)(1), which was contained in OSHA inspection number 666778, citation number 1, item number 1 and was affirmed as a final order on November 27,2012, with respect to a workplace located at 1747 Village Square, Flemming Island, Florida.
