29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) Roof, new residential construction located at 2950 Manorview Lane, Milton, GA 30004: Employees operating Hitachi Framing and Finish Nail Guns and a DuoFast Siding Nail Gun were exposed to struck-by hazards as protective eyewear was not used while operating these pneumatic tools at 120 PSI. Employees were exposed to flying debris and fasterners as safety glasses or goggles were not worn.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  (a) West side, new residential construction located at 2950 Manorview Lane, Milton, GA 30004. Employees installing cornice boards from a pump jack scaffold system at heights of approximately 18-feet, 1-inch were not protected from falls to the lower level as a fall protection system was not provided.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Roof, new residential construction located at 2950 Manorview Lane, Milton, GA 30004: Employee was exposed to falls of approximately 18-feet, 1-inch while installing cornice boards and siding from a 8/12 pitched roof with no fall protection system in place.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) West side, new residential construction located at 2950 Manorview Lane, Milton, GA 30004: The ladder provided was not secured to the structure and did not extend 3-feet above the working surface to allow for safe access on and off the pump jack scaffold and the roof surface. Employees accessing this ladder from the pump jack scaffold platform were exposed to falls of approximately 18-feet, 1-inch to the lower levels.
