29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  a) 26 Galaxy Pass, Sutton, MA : On or about November 16, 2015, employees were using an aluminum extension ladder to reach the roof level that did not extend the required 3 feet past the upper landing surface.   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a) 26 Galaxy Pass., Sutton, MA: On or about November 16, 2015, the employees were walking across an open joist bay to reach the working area because the only access ladder was set up so that they had to cross the open truss joists to get to their work location.   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) 26 Galaxy Pass., Sutton, MA: On or about November 16, 2015, the employees were decking the roof of this building without any fall protection more than 16 feet above the ground below.  Edgewater Construction Incorporated was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b) 1, which was contained in OSHA inspection number 1016973, citation number 1, item number 1and was affirmed as a final order on  January 8, 2015 with respect to a workplace located at23-35 Durham St., Durham, NH.
