29 CFR 1910.132(a):  Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and/or maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:  a) Wal Mart Store #2629 100 Valley Parkway., Whitinsville, MA: On or about November 12, 2015, an employee had his right foot crushed by a loaded pallet jack that rolled over his unprotected foot when he was unloading a tractor trailer of goods for the store. According to documents provided by the employer, this hazard (foot impact hazards) is recognized, but no PPE is required or provided to the employees to mitigate this hazard.   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
