29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   A). On or about 11/03/15, at the site of 32-37 105th Street. Corona, NY   Employees installing first floor at the site were in danger of head injury from falling objects such as, but not limited to tools and roof materials from the above. Protective helmets were not worn.    ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which were used by employees did not have approved ground fault circuit interrupters for personnel protection:                                                                                                                               A).       On or about 11/03/15, at the site 32-37 105th Street. Corona, NY    Tools connecting to receptacle outlets on a Powermate generator was not equipped with ground fault circuit interrupters.  ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.

Corona, NY  Employees installing a floor frame were exposed to the hazard of falling approximately 11 ft. to the ground below.  ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
