29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: (a) On or about November 10 ,2015, and at times prior, Midwest Roofing & Custom Metals, did not ensure that employees engaged in roofing activities were protected from falling through holes, more than six feet above lower levels. Employees of Midwest Roofing & Custom Metals were exposed to fall hazards in excess of 23 feet while repairing openings on a flat roof, without the benefit of fall protection systems. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
