On or about November 5, 2015, at the worksite located at 5115 Belinder Rd., Westwood, KS, employees making connections to a pole did not keep loose tie wire away from an exposed energized conductor.

29 CFR 1910.269(a)(4): Existing characteristics and conditions of electric lines and equipment that are related to the safety of the work to be performed were not determined before work on or near the lines or equipment is started: On or about November 5, 2015, at the worksite located at 5115 Belinder Rd., Westwood, KS; employees were exposed to arc flash hazards in that the circuit voltage of 12,400 volts AC was not discussed during the Pre-Job briefing, and the circuit voltage was not recorded on the Pre-Job briefing and Job Hazard Analysis number 017984 dated 11-5/15 for job number 1652532.
