29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact:   a.)    SW Commercial Roofing, LLC, 205 West 10th Street, Erie, Pa 16501, on or about November 19th, 2015, and times prior thereto:  Employees performed work operations including, but not limited to, spray foam application and operations with an Elastospray 8000A Isocyanate (Polymethylene Polyphenylisocyanate).  Employees were not utilizing gloves during all work operations with the isocyanates as evidenced by the fact that, aromatic isocyanates were present on the employees hands and/or skin based on observations and dermal sampling.

SW Commercial Roofing, LLC, 205 West 10th Street, Erie, Pa 16501, on or about November 19th, 2015, and times prior thereto:  Employees were periodically exposed to hazards including, but not limited to, chemical exposure hazards to the hands and/or body during work operations with Isocyanates (Elastospray 8000A Isocyanate) onsite.  Employees were not trained in the proper use, storage, care, and limitations of personal protective equipment.
