29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    A)    Taiho Corporation of America, Inc., located at 194 Heritage Drive, Tiffin, OH 44883: On or about November 13, 2015, the employer did not ensure that employees utilize established energy control procedures to control hazardous energy of equipment such as, but not limited to, the Facers on Line 3 and Line 4. Employees did not control all hazardous energy sources such as, but not limited to, electrical and pneumatic before performing activities such as, but not limited to, changing tooling and unjamming the machines. Employees were exposed to amputation hazards.
