29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:  Commercial building under construction; North Side, Genie Aerial Lift (Model: S40, Equipment Rental Number: 115923), On or about November 18, 2015, employees were exposed to fall hazards of approximately 10 feet in height to the unpaved ground below, when patching concrete from an aerial lift without fall protection.
