29 CFR 1910.134(h)(4): Respirators that failed an inspection or were otherwise found to be defective were not removed from service and discarded or repaired:     On or about November 9, 2016, the air-supplied respirator, that employees were using to enter the railcars, had a defective air-hose coupling which caused it to disconnect from the respirator.  The employer used duct-tape to hold it together while employees sand-blasted the interior of the railcars.  The employees were exposed to high levels of silica when the hose became uncoupled.     Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
