29 CFR 1910.147(d)(2):     The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment:    On the Large Truck Line the employer is failing to ensure that employees are not exposed to amputation hazards when changing out drums and scraping excess caulk from the Graco L20c caulk pump.  Employees was changing out an empty 5 gallon bucket of caulk. In order to accomplish this task he needed to scrape caulk from the platen and priming plate.  The pump unexpectedly cycled causing the priming plate that was attached to a feed tube to move upwards toward the platen.  This action and resulting pinch point caused the employees right index finger to become impinged resulted in an amputation.  This was most recently documented on November 5, 2015.
