On or about November 17, 2015, in a 9-foot deep excavation, four employees performing concrete reinforcement operations were exposed to an overhead hazard, in that the employees were not wearing protective helmets, thereby exposing themselves to being struck-by rocks and pieces of pavement located at the edges of the excavation, pipes, tools or other debris located on top of the culvert boxes.
