Safeway Packaging, Inc. located at 300 White Mountain Dr. New Bremen, OH; On November 10, 2015, the employer did not ensure employees were protected while performing setup and cleaning activities on the McKinley Rotary Die Cutter Press (RDC3). An employee was using an air hose to clean out scrap from underneath a conveyor, when another employee started up the conveyor, resulting in a degloving of the left wrist.

Employees were required to rewire the 480V conveyor motor and did not wear the required face, eye, hand, and body protection, exposing them to electrical shock and arc flash hazards when verifying 0 energy with a meter. Safeway Packaging, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.335(a)(1) which was contained in OSHA inspection number 31378713, citation number 1, item number 2a and was affirmed as a final order on 12/8/12 with respect to a workplace located at 300 White Mountain Dr. New Bremen, OH.
