Safeway Packaging, Inc. located at 300 White Mountain Dr. New Bremen, OH; On or about November 18, 2015, the employer did not assure the purpose and function of the energy control program were understood. Employees are required to perform cleaning and setup activities on equipment such as, but not limited to the McKinley Rotary Die Cutter and are exposed to hazards associated with accidental startup of the equipment.

29 CFR 1910.147(d)(2): An orderly shutdown of machine or equipment was not utilized to avoid any additional or increased hazards to employees as a result of equipment stoppage:     a.)    Safeway Packaging, Inc. located at 300 White Mountain Dr. New Bremen, OH; On November 10, 2015, the employer did not ensure employees were protected while performing setup and cleaning activities on the McKinley Rotary Die Cutter Press (RDC3). An employee was using an air hose to clean out scrap from underneath a conveyor, when another employee started up the conveyor, resulting in a degloving of the left wrist.

Safeway Packaging, Inc. located at 300 White Mountain Dr. New Bremen, OH; On or about November 18, 2015, the employer did not ensure employees were protected while working on the McKinley Rotary Die Cutter #3. Employees were required to perform rewiring work on a motor and did not place a lock on the 480V disconnect before the start of work.

Safeway Packaging, Inc. located at 300 White Mountain Dr. New Bremen, OH; On or about November 18, 2015, the employer did not ensure employees were trained in and familiar with electrical safety related work places before rewiring a motor on the McKinley Rotary Die Cutter #3. Employees were required to perform electrical work on 480V and did not have any training on Electrical Safety Related Work Practices.    Safeway Packaging, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.332(b)(1) which was contained in OSHA inspection number 31378713, citation number 1, item number 1a and was affirmed as a final order on 12/8/12 with respect to a workplace located at 300 White Mountain Dr. New Bremen, OH.

Safeway Packaging, Inc. located at 300 White Mountain Dr. New Bremen, OH; On or about November 18, 2015, the employer did not ensure employees were protected while working on the McKinley Rotary Die Cutter #3. Employees were required to rewire the 480V conveyor motor and did not wear the required face, eye, hand, and body protection, exposing them to electrical shock and arc flash hazards when verifying 0 energy with a meter.      Safeway Packaging, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.335(a)(1) which was contained in OSHA inspection number 31378713, citation number 1, item number 2a and was affirmed as a final order on 12/8/12 with respect to a workplace located at 300 White Mountain Dr. New Bremen, OH.
