29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket: (a) On November 18, 2015 the worksite located at 3211 Gardner Ave Kansas City, MO 64120. The employer failed to provide training for the employee on maintaining two feet on the floor of the basket of the aerial at all times. The employee was exposed to a fall hazard in excess of 19 feet when he climbed up onto the railing of the aerial lift in order to adjust and install new ventilation.
