29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): On or about November 11, 2015, employees working from a fabricated steel frame scaffolding system were doing so with the outrigger (personnel) platforms not fully planked.

29 CFR 1926.454(c): When the employer has reason to believe that an employee lacks the skill or understanding needed for safe work involving the erection, use or dismantling of scaffolds, the employer shall retrain each such employee so that the requisite proficiency is regained. On or about November 11, 2015, employees lacked the skill or understanding to provide a safe work environment when performing scaffold duties such as but not limited to: raising the outrigger (personnel) platforms.
