29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) For employees working at 319 Poinsettia Ave in Sebring, as observed on or about 17 November, 2015. Employees were installing roll roofing materials on a low sloped roof. The top landing surface to ground height measured approximately 14 feet and the employees had no fall protection system in place.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) For employees working at 319 Poinsettia Ave in Sebring, as observed on or about 17 November, 2015. Employees were using an extension ladder to access the roof. The top of the extension ladder measured approximately 18 inches above the upper landing surface. The top landing surface to ground height measured approximately 14 feet.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. a) For employees working at 319 Poinsettia Ave in Sebring, as observed on or about 17 November, 2015. Employees were using electrical power tools on the roof that were not protected by a working Ground Fault Circuit Interrupter. The Ground Fault Circuit Interrupter was not grounded and did not open the circuit when tested.
