Inc., on or about 11/16/15:  At the establishment there was no one person who was adequately trained to render first aid in the absence of an infirmary, clinic or hospital.  On 11/16/15 and employee sustained a head injury while cleaning wood debris from under the debarker log deck.  Emergency medical services responded to this incident 45 minutes to an hour from the occurrence.  b)  Two Rivers Timber Co. Inc., on or about 2/3/16:  At the establishment there was not one person who was adequately trained to render first aid in the absence of an infirmary, clinic or hospital.  On 2/3/16 an employee suffered severe hand and finger amputations.   The injured employee was transported in a personal vehicle to meet Rural Metro Ambulance due to the uncertainty of their response time.
