Brad Miller dba Millers Plumbing and Excavating, 5140 West Ridge Road, Erie, PA 16506, on or about November 19, 2015 and times prior thereto:  Employees were utilizing electrical equipment including, but not limited to, a concrete mixer for masonry operations.  The mixer was powered by plugging and unplugging the mixer from the energized flexible extension cord.  The concrete mixer's plug was equipped with a cardboard cover dead front.  The cardboard dead front was not suited for the conditions encountered on construction sites.  B.) A  written demolition plan was not developed for the demolition work.  B.)  Brad Miller dba Millers Plumbing and Excavating, 5140 West Ridge Road, Erie, PA 16506, on or about November 19, 2015 and times prior thereto:  Employees were performing demolition operations including, but not limited to, hand removing a block basement foundation wall.  A written demolition plan was not developed to identify and control hazards associated with a potential collapse of the single-sided excavation that was located next to the basement wall.  A demolition plan was not established to cover control measures to prevent the potential collapse of the excavation and  the forces an excavation collapse would exert on the shoring and the wall.
