This was most recently evident during the inspection of the facility located at 532 Old Sawmill Road, South Sioux City, NE, where the employer has employees operating forklifts as a means of moving material and product throughout the facility without wearing the seatbelt.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:     The employer is failing to protect employees from caught between and amputation hazards associated with operating and maintaining equipment that requires locking out stored energy without developing and implementing written procedures.  This was most recently evident during the inspection of the facility located at 532 Old Sawmill Road, South Sioux City, NE; where the employer has employees applying locks as means of securing hazardous stored energy without having developed and implemented written procedures that outline the step by step process necessary to safely de-energize that equipment.  Instances include but are not limited to:           i.        Employee locking out the chain conveyor has never seen or heard of written specific procedures.        ii.        Three months earlier employees locked out and worked on the Pallet Dismantling Machine without having written procedures available.       iii.        The day prior to the inspection an employee locked out and worked on both the Gang Saw and the Trim Saw, without having written procedures available.
