29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) construction site, Reservoir Rd. Water Supply, Albion, PA. On or about November 13, 2015, and times prior thereto, workers were not protected from a 12-foot fall into the pump-house pit. A portion of the temporary guardrail had been removed, leaving a 57-inch opening.

29 CFR 1926.405(a)(2)(ii)(B): Runs of open conductors were not located where the conductors would not be subject to physical damage: a) construction site, Reservoir Rd. Water Supply, Albion, PA. On or about November 13, 2015, and times prior thereto, the inner wires of the temporary wiring to power the lighting inside the pump-house were exposed to physical damage including, but not limited to, pulling/ snagging, stepping on, and dragging ladders over them. The wires were routed down the wall and across the floor. An open splice that was associated with the wires was positioned on the floor.
