29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:  On or about 11/17/15  Location: 85 South Road, White Plains, NY 10603   a) Employees used a damaged 100 foot orange extension cord to power an Makita circular saw. The outer insulation was cut and torn for a length of six inches exposing the insulated inner conductors.
