29 CFR 1910.179(h)(2)(v):  Rope clips attached with U-bolts did not have the U-bolts on the dead or short end of the rope:  Near the north wall of the shop:  On or about November 19, 2015, and at times prior thereto, employees were exposed to the hazards of a malfunctioning U-bolt securing a cable supporting the transformer, as the bolt was not on the short end of the rope.

These inspections include the requirements of paragraph (j)(2) of this section, to include hooks, hoist chains, ropes, and all functional operating mechanisms, and in addition, the following items, such as bolts, rivets, drums, and damaged components:  Near the north wall of the shop:  On or about November 19, 2015, and at times prior thereto, employees were exposed to the hazards of a malfunctioning overhead Contrx crane, in that it had not been inspected as required, while operating a spot welder which it supported.
