29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about November 19, 2015 and at times prior thereto, at this establishment site, employees were not trained in the following: A. The employees who were required to install metal shelving were not trained in how to recognize fall hazards. B. The employer had not trained employees on how to utilize a fall protection system in shelving operations.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. At this warehouse, employees erecting storage shelving working at heights over 15 feet did not have a form of fall protection to keep them from falling to the concrete below.
