29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about November 10, 2015: Employees engaged in die repair activities using the remote, pneumatic valve actuator, in the Stamping Maintenance Department were exposed to a caught-in hazard in that machine guarding was not provided while energizing a slide cam.

29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinders were designed to accept caps, were not always in place, hand-tight, except when cylinders were in use or connected for use: Employees working in the Stamping Maintenance Department in the vicinity of door #58 were exposed to injury from a fire or explosion hazard. The compressed gas cylinder of acetylene was not provided with a valve protection cap.

29 CFR 1910.253(b)(4)(i): Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire: Employees working in the Stamping Maintenance Department in the vicinity of door #58 were exposed to injury from a fire or explosion hazard. The compressed gas cylinder of oxygen was stored with a compressed gas cylinder of acetylene.
