29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms:   A) South west section of building, on or about November 18,  2015, an employee was working from an out rigger above the third level of a 4 tier welded frame scaffold that did not have a guardrail system on the end of the out rigger platform, exposing employees to a 20 foot  fall hazard.      B) South west section of building, on or about November 18,  2015, an employee was working from a welded frame scaffold that was missing a mid-rail on the third level platform, exposing the employee to an 18 foot fall hazard.     C) North east section of building, on or about November 18, 2015, an employee was working from the second level platform of a welded frame scaffold that did not have a mid-rail as part of the guardrail system, exposing the employee to a 13 foot fall hazard.
