29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  Jobsite: Employees were exposed to a fall of approximately eighteen (18) feet, while performing roofing operations, with no fall protection.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder:  a) Front of building: An aluminum ladder was utilized to access the roof that was not long enough to extend three feet above the landing.  The ladder was not secured to a rigid support and a grasping device was not provided exposing employees to a fall of approximately nine (9)  feet to the grass below.  b) Rear of building: An aluminum ladder was utilized to access the roof that was not long enough to extend three feet above the landing.  The ladder was not secured to a rigid support and a grasping device was not provided exposing employees to a fall of approximately nine (9) feet to the concrete below.
