29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:    a.  On or about November 13, 2015, at the North Tower, CSW Tower and Water Tower, employees engaged in the installation of new wireless equipment at high elevations had not been trained by the employer in fall protection procedures and tower climbing safety, exposing employees to fall hazards.
