On or about November 18, 2015, an employee who was operating a 'Genie" brand aerial lift was not protected by a fall protection harness or body belt with the lanyard attached to the boom or basket and exposed to an approximate 25-foot fall to the lower level below. b. On or about November 18, 2015, an employee who was operating a 'JLG Lift" brand aerial lift was not protected by a fall protection harness or body belt with the lanyard attached to the boom or basket and exposed to an approximate 16-foot fall to the lower level below.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. a. On the roof at the worksite, on or about November 18, 2015, seven (7) employees and the company owner were engaged in roofing activities on low-slope roof with unprotected sides and edges and were not protected by any form of fall protection and exposed to falls hazards greater than twenty-five (25) feet to the lower surface below.
