29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On November 19, 2015 an employee was not provided with fall protection while working on a steep commercial roof, exposing the employee to a fall of 9.1 feet. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about November 19, 2015 an employee working at heights was not trained to recognize or minimize the hazards associated with a fall, exposing the employee to fall of 9.1 feet.
