OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards from non-rated lifting devices.        The employer did not ensure that the lifting device, (a spreader bar) used outside with the 15 Ton crane was rated, stamped and inspected for the employees to use for rigging up and/or moving material; exposing employees to struck by hazards.

Employees were exposed to caught by and amputation hazards associated with machines that have point of operation, ingoing nip points, rotating parts, flying chips and sparks.        a)  60 ft. ton "Pullmax" press located in Bay 7, had no barrier guard to the rear to protect employees from entry.      b)  the electric winch and its cable line located in Bay 5 had no guards in place and had a damaged cable.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws.     Employees were exposed to electrical shock hazards while using electrical extension cords located in Bay 1 that were not protected from direct pull to the terminal and internal wiring by a strain relief mechanism.  The outer sheathing was pulled away from the terminal end exposing the internal wiring.
