The specific procedures for equipment did not include a specific statement of intended use of the procedure; did not include specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; did not include specific information regarding the removal and transfer of lockout devices or tagout devices and the responsibility for them; specific procedural steps for testing machines and equipment to verify the deenergization; specific requirements for notification of affected employees prior to work commencing; and specific procedures for reenergization of the machines.
