29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a) Pro Process, LLC / 1283 Brusselles Street, Saint Marys, PA 15857: On or about November 18, 2015 and times prior thereto, employees periodically performed servicing and maintenance on lathes and milling machines including, but not limited to, DMG Mori J1 Wasino lathe, Tecno Wasinos, Mor Seiki C1 1500 lathe, DMV-400s, DVC-400, Lynx 220, Lynx 210, Lynx 200, Amada J1 as well as other equipment associated with production. The specific procedures for equipment did not include a specific statement of intended use of the procedure; did not include specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; did not include specific information regarding the removal and transfer of lockout devices or tagout devices and the responsibility for them; specific procedural steps for testing machines and equipment to verify the deenergization; specific requirements for notification of affected employees prior to work commencing; and specific procedures for reenergization of the machines.

29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed: a) Pro Process, LLC / 1283 Brusselles Street, Saint Marys, PA: On or about November 18, employees were potentially exposed to hazards including but not limited to eye hazards from particles from cleaning parts from the lathes and mills as well as chemical exposure to the eyes, face, hands and arms and trunk from cutting fluid for the lathes and mills. The employer was not able to provide a written certification of the workplace hazard assessment.
