29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    On or about November 18, 2015, the employer did not establish and implement a  written respiratory protection program with required worksite specific procedures and elements  for employees who are required to wear 3M tight fitting face piece particulate respirators in areas such as the powder coat booth/spray area.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that they establish and implement a written respiratory protection program that includes the requirements of 29 CFR 1910.134(c)(1)(i-ix).

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  On or about November 18, 2015, the employer did not ensure that employees that are required to wear respirators were medically evaluated to prevent employee injury.  Employees in the powder coat shop were exposed to respiration hazards when wearing 3M tight fitting face piece particulate respirators while engaging in spraying operations.
