29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired. a. At the worksite where employees plugged in extension cords and electrically powered portable tools, inspections were not conducted before each days use, thereby exposing employees to an electrical contact hazard.

29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. a. In the smokehouse/chiller room where employees plugged extension cords into a temporary electrical receptacle, the ground prong was missing on an extension cord, thereby exposing employees to an electrical contact hazard. b. In the smokehouse/chiller room where employees plugged a portable band saw into an extension cord, the ground prong was missing on the band saw, thereby exposing employees to an electrical contact hazard.
