29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired. a. At the worksite where employees used extension cords and a corded drill, inspections of the equipment were not conducted before each days use, thereby exposing employees to an electrical contact hazard.

29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. a. In the main entrance hallway where employees plugged an extension cord into the temporary electrical receptacle to provide power to the mud mixer, the ground prong on the male end of the extension cord was missing, thereby exposing employees to an electrical contact hazard.
