29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. a. In the south side first floor of the amenity building where employees walked through the building, a hole was cut into the floor that was not protected, thereby exposing the employees to a 5 foot, 2 inch fall hazard.
