29 CFR 1926.451(f)(3): Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity. a. At the worksite where employees were laying block near a damaged guardrail section, an inspection of the scaffold for defects was not conducted before each shift, thereby exposing employees to a 32 foot fall hazard.

29 CFR 1926.451(g)(4)(iv): When midrails are used, they shall be installed at a height approximately midway between the top edge of the guardrail system and the platform surface. a. On the south east corner of the scaffold where two sections of scaffolding met, the midrail was not installed midway between top rail and the platform surface, thereby exposing the employees to a 32 foot fall hazard.
