29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: Unit 6, 8 Burning Tree Road, Great Barrington, MA: On or about November 20, 2015 employees were using an aluminum portable, extension ladder to access the roof and its rungs were not extended at least three feet above the upper landing.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). Roof, Unit 6, 8 Burning Tree Road, Great Barrington, MA: On or about November 20, 2015 employees were observed installing shingles on a residential roof over 12 feet above the next lower level (ground) and were not protected by either guardrail systems, safety net system, or personal fall arrest system. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
