29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    a) Facility, 1048 Sandy Hill Road, Irwin, PA:  On or about November 20, 2015, an employee was required by the employer to wear a respirator while using the KI abrasive cut off saw while cutting tubes and the employer does not have a written respiratory program.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  a) Facility, 1048 Sandy Hill Road, Irwin, PA:  On or about November 20, 2015, an employee was exposed to electrical hazards while heat treating a piece of tool steel in the furnace because a 4" X 4" opening on the bottom front of the 220 volt Heat Ray Tool Room Furnace exposed the wiring to the Industrial Control Transformer was not covered.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  a) Jobsite, 1048 Sandy Hill Road, Irwin, PA:  On or about November 20, 2015, the employer did not perform a workplace hazard assessment of the facility to determine what PPE was necessary and an employee was injured while working with a steel plate that fell on his foot and broke his toe.  The employee was not wearing work boots or steel toed boots that would have provided more protection than the tennis shoes he was wearing while handling heavy objects.
